Identify your specific issue:

Did you bill a non-family-planning procedure for a Healthy Connections Checkup and Family Planning beneficiary?

  • Make sure the Medicaid ID number reflects the patient and not, for example, another family member.
  • Only bill procedures indicated for Healthy Connections Checkup and Family Planning beneficiaries.

Did you include the correct diagnosis code, procedure code, and/or modifier "FP" on the claim?
